We hate to see you go but we wish you the best in your future endeavors!
Follow these steps to ensure smooth processing of your departure:
- Resignation Letter: Provide your supervisor with a resignation letter and send a copy to Human Resources.
- Departure Clearance Form: Obtain and complete the Employee Departure Clearance Form (will require signatures from various offices as you return your ID, keys, parking permit, P-card, etc.).
- Departing Employee: Please complete the top portion of the form and enter your signature down at the bottom. You should then forward this form to your supervisor for signature.
- Supervisor: Once both the employee and supervisor’s signatures have been obtained, the supervisor should e-mail the form to Monica Arrendale. Monica will then send it out for the rest of the signatures.
- Exit Interview: Schedule an exit interview with Employee Relations.
- COBRA: Follow up with Human Resources concerning continuation of benefits under COBRA.
- Find out more about COBRA here.
- Once your termination information has been uploaded to the OneUSG Connect benefit portal, you will need to go into the portal and enroll in COBRA if you choose to continue your benefits.
- Retirement Accounts: Follow up with Human Resources concerning your retirement accounts.
- Vacation Payout: Contact payroll concerning your vacation payout.
- Sick Leave: There is no payout for sick leave earned but not taken. However, employees retiring with the Teachers Retirement System (TRS) may use 60 or more days of accumulated sick leave to earn additional service time.
- Flexible Spending Account/Health Savings Account: Contact Human Resources to find out what happens to your flexible spending accounts and/or Health Savings Account when you leave.
